Central and Eastern Sydney PHN (CESPHN) has been funded by the Federal Department of Health to support activities within primary health care based on community needs and regional planning.
With this in mind CESPHN is committed to supporting a positive comprehensive, person-centred integrated system of care for the people of Norfolk Island. CESPHN aims to achieve this through strong collaboration with the Norfolk Island Health and Residential Aged Care Service (NIHRACS), local service providers and most importantly the local community.
To further this aim, CESPHN developed a draft Health Promotion Plan (Plan) based on the Norfolk Island Needs Assessment and further feedback and consultation with the people of Norfolk Island. CESPHN acknowledges that the Norfolk Island (NI) community have already delivered many activities in the promotion of health and wellbeing and that it is hoped that further activities will support, empower and build individual and community understanding and resilience.
The Plan includes a range of health promotion activities that focuses on enhancing community health and wellbeing knowledge, creating health awareness, building positive attitudes towards health and wellbeing and encouraging help-seeking.
CESPHN will serve as a commissioning (funding) organisation supporting the implementation of the Plan’s activities and will work in collaboration with providers to action activities and identify measures of success for each stage. This may include methods of implementation, reporting, monitoring and evaluation.
The development by CESPHN of the Plan is a key element of the broader NI Activity Work Plan 2016 – 2018 . The Plan offers the opportunity to embed partnerships with Norfolk Island Health and Residential Aged Care Service (NIHRACS) and community groups to make optimal use of resources and ultimately deliver improved access, health outcomes and experiences of care for Norfolk Island communities and their visitors.
